Managing a Respiratory Disorder: Tips for Patients and Families

Unfortunately, respiratory disorders can affect your respiratory function (the ability to breathe) and your pulmonary function (how well lungs work). Importantly, the respiratory system takes in oxygen from the air and removes extra carbon dioxide, a vital task needed for every organ in your body. Interestingly, respiratory disorders, often referred to as chronic lung diseases, usually develop slowly but can worsen over time. Managing a respiratory disorder must involve the patient, his/her family, and a qualified team of medical professionals.

If left untreated, respiratory disorders (lung diseases) can lead to complications, problematic symptoms, and life-threatening conditions. In fact, lung disease is the 3rd leading cause of death in the US. Moreover, lung disease and other breathing issues are leading causes of death in infants.

These conditions demand more than just medication; they require comprehensive, consistent, and compassionate care at home and in clinical settings. While doctors and respiratory therapists play critical roles, patients and their families have an important role as well.

Effective respiratory care is not a one-size-fits-all solution. Each patient’s journey is unique, requiring a combination of medical knowledge, empathy, communication, and adaptability from everyone involved. This article aims to provide patients and their families with the tools and insights needed while managing a respiratory disorder.

Types of respiratory disorders and treatments.

Each type of disease impacts different parts of your respiratory system in different ways. Understanding which type of disease you have (or your loved one) can help you get the best outcome possible. Importantly, you can have one or all of these types of respiratory disease.

Below are some common respiratory disorders.

Chronic obstructive pulmonary disease (COPD)

Importantly, COPD can refer to two conditions:

  • Emphysema, which happens due to damage to the tiny air sacs in the lungs, affecting your ability to move air in and out of your body.
  • Chronic bronchitis, which leads to persistent inflammation or irritation of the airways and the formation of thick mucus.

Risk factors include smoking, air pollution, and occupational exposures.

COPD treatments.

There’s no cure for COPD, but you can manage symptoms with:

  • Medications to open the airways.
  • Pulmonary rehabilitation.
  • Oxygen therapy.
  • Surgery.

Asthma

The exact cause of asthma is unknown, but asthma is more likely to occur in people with:

  • Other close family members who have asthma.
  • Other allergy-related conditions, like hay fever and eczema.
  • Exposure to environmental allergens or irritants.

In some cases, asthma attacks can be serious and even lead to death.

Asthma treatment.

Asthma patients generally use medications that help relax the airways, making breathing easier. These medications are typically given via an inhaler.

Obstructive sleep apnea (OSA)

OSA occurs if you have a condition that blocks your airways during sleep. It’s more common among people who are older, obese, or have a family history of OSA.

Unfortunately, OSA is linked to a higher risk of a variety of other health problems, including:

  • Cardiovascular disease.
  • Type 2 diabetes.
  • Depression.
  • Dementia.

OSA treatment.

Treatment often includes the use of a CPAP device that helps keep airways open while you sleep.

Pulmonary hypertension.

Pulmonary hypertension occurs when the blood pressure in the blood vessels of your lungs is too high. Unfortunately, it can lead to serious health problems, including blood clots, heart failure, and arrythmias.

Although the cause of pulmonary hypertension is often unknown, some medical conditions like heart failure, lung disease, and blood clots can contribute.

Pulmonary hypertension treatment.

The treatment can include:

  • Medications to lower blood pressure and prevent blood clots.
  • Oxygen therapy.
  • Surgery.
  • Treating the underlying cause.

Interstitial lung disease (ILD)

Types of restrictive lung disease include:

  • Pulmonary fibrosis or Idiopathic Pulmonary Fibrosis (IPF).
  • Sarcoidosis
  • Work-related lung diseases (see below).

Although many causes of ILD remain unknown, known causes include:

  • Exposure to hazardous substances.
  • Autoimmune conditions like sarcoidosis and rheumatoid arthritis.
  • Specific medications or medical treatments.

ILD treatment.

There’s no cure for ILD. However, treatment including medications can reduce inflammation in the lungs. Additionally, oxygen therapy and pulmonary rehabilitation can help patients feel better. Doctors may recommend a lung transplant in very severe situations.

Lung cancer.

Lung cancer is when cancer begins in your lungs.

Risk factors include:

  • A family history of lung cancer.
  • Secondhand smoke exposure.
  • Occupational exposures to harmful substances.
  • Radiation exposure, including radon at home or receiving previous radiation therapy.
  • Long-term exposure to air pollution.

Lung cancer treatment.

Lung cancer treatment depends on the type and stage of the cancer. Treatment may include one or more of the following treatments:

  • Surgery.
  • Chemotherapy.
  • Radiation therapy.
  • Targeted therapy.
  • Immunotherapy..

Occupational lung diseases.

  • COPD.
  • Work-related asthma.
  • Asbestosis.
  • Mesothelioma.
  • Silicosis.
  • Pneumoconiosis (black lung disease).
  • Hypersensitivity pneumonitis.

Treatment of occupational lung diseases.

The type of treatment depends on the type of lung disease. Treatment may involve medications, oxygen therapy, pulmonary rehabilitation, or a lung transplant.

Other respiratory diseases.

Other examples of chronic respiratory diseases that are less common include:

  • Cystic fibrosis.
  • Bronchiolitis obliterans (popcorn lung).
  • Alpha-1 antitrypsin deficiency.
  • Lymphangioleiomyomatosis (LAM).
  • Bronchopulmonary dysplasia.
  • Pulmonary alveolar proteinosis.
  • Primary ciliary dyskinesia.

Additionally, there are several acute (short lasting) respiratory conditions, including pulmonary embolism, collapsed lung, and infections that can cause a variety of respiratory problems, including:

  • Bronchitis.
  • Bronchiolitis.
  • Pneumonia.
  • Acute respiratory distress syndrome (ARDS).

Symptoms of lung diseases.

Symptoms can include:

  • Persistent cough, sometimes with blood.
  • Wheezing.
  • Shortness of breath.
  • Tightness in chest or chest pain.
  • Fatigue.
  • Increased production of mucus or phlegm.
  • Hoarse voice.
  • Reduced appetite.
  • Unexplained weight loss.
  • Dizziness.
  • Heart palpitations.
  • Weakness.
  • Swelling in the lower body.
  • Skin, lips, or nails that have a blue, gray, or pale color.

Symptoms of obstructive sleep apnea include:

  • Breathing that stops briefly during sleep.
  • Loud snoring.
  • Gasping or choking noises while sleeping.
  • Excessive daytime sleepiness or fatigue.

  • Shortness of breath.
  • Fast breathing.
  • Rapid heartbeat.
  • Coughing or wheezing.
  • Headache.
  • Feeling confused or disoriented.
  • A blue coloration to the skin, lips, or fingernails.

Importantly, hypoxemia is a serious condition. Seek immediate medical attention if you or someone else shows signs of hypoxemia.

Risk factors for respiratory disorders.

The specific risk factors for respiratory disorders can vary depending on the type of disease. However, many respiratory disorders share some risk factors.

For instance, smoking is a major risk factor for COPD and lung cancer. Smoking is also a risk factor for other disorders.

Additionally, exposure to a variety of hazardous substances can increase the risk of respiratory conditions such COPD, asthma, ILD, occupational lung disease, and lung cancer. Examples of hazardous substances can include:

Some other health conditions increase the risk of respiratory diseases, including obesity and allergies.

Additionally, bacterial, viral or fungal infections can lead to pneumonia.

And genetic factors can play a role in the development of respiratory diseases, as can a history of frequent lower respiratory infections during childhood.

Key elements of managing a respiratory disorder.

Education and training.

This includes learning:

  • How to use inhalers, nebulizers, and oxygen therapy machines.
  • Early warning signs of worsening symptoms.
  • Medication schedules and dosage instructions.
  • Cleanliness and hygiene protocols to avoid infections.

Hospitals and care centers often provide workshops or guides which can provide essential support to patients and families. Talk to your doctor about available resources and take full advantage of them.

Environmental control.

Air quality can significantly impact a patient’s respiratory health. Patients and families should aim to create a home environment that supports healthy breathing by following these suggestions from American Lung Association:

Identify and eliminate sources of particulate matter.

  • Avoid burning things in your home, including fireplaces, wood stoves and candles.
  • While cooking, use an exhaust fan that vents outdoors.
  • Avoid chemicals such as air fresheners, incense, sprays and toxic cleaning products.
  • Use door mats and remove shoes upon entering the home to reduce dirt tracked inside.
  • Use a high-efficiency vacuum cleaner (called HEPA) or a central (whole-house) vacuum cleaner to frequently clean carpets and floors. Additionally, mop hard floors often.
  • Wear an N95 particle mask in smoky or dusty conditions, such as when using a leaf blower or lawn mower.
  • Avoid activity outdoors when outdoor pollution levels are high.
  • When enjoying outdoor activities, stay away from areas with harmful particle pollution, such as busy roads.
  • Use electric instead of gas-powered lawn and garden equipment.

Improve ventilation.

Therefore, open your doors and windows for 15 minutes each day to reduce indoor pollutants. However, do not keep your doors and windows open on days with poor outdoor air quality, if you live close to busy highways or factories with high emissions, or if there is a wildfire nearby. Check the current and forecasted air quality levels for your city at AirNow.gov.

Clean the air.

Importantly, change the filter about every 30 days or when it looks dirty.

Additionally, consider using a portable HEPA air cleaner. Importantly, run the portable air cleaner with all doors and windows closed for maximum efficiency. Large areas and homes may need more than one air cleaner.

Interestingly, research found that people with COPD who consistently used HEPA air cleaners for six months experienced fewer symptoms, had improved lung function, used their rescue medication less and reported an improved quality of life.

Consider your options and talk to your doctor before using air purifiers.

Newer technology in central and portable air cleaners should be used only after careful consideration, as some may have potentially adverse health consequences, including ozone production or the creation of unintentional pollutants. Importantly, many of these newer technologies are still being studied and their effectiveness is unknown. Moreover, there are no standards for testing these new technologies in the US.

Medication management.

  • Keep a checklist or digital reminder system.
  • Use pill sorters, which will make it easier to take the right pills at the right time, as well as act as a visual reminder that pills were taken.
  • Ensure prescriptions are filled on time.
  • Monitor for side effects or interactions.
  • Communicate changes, including side effects and worsening health, to the healthcare provider promptly.

Home monitoring.

Patients with advanced respiratory conditions often require home monitoring, which can include the use of:

  • A pulse oximeter to track oxygen saturation.
  • A spirometer to measure lung function.
  • Wearables such as smart watches or step counters.
  • Smartphone apps to monitor symptoms.
  • Electronic inhaler monitors that track medication delivery.
  • Remote or virtual exercise tests.

Other monitoring may include making notes of:

  • Respiratory rate and effort.
  • Changes in sleep quality or energy levels.
  • Daily symptoms.

Nutritional support.

Nutrition plays an important role in respiratory health. Importantly, certain foods can reduce inflammation and boost lung function. Patients with respiratory disorders should talk to their doctors for tips.

If the patient has difficulty swallowing or maintaining weight, consult with a registered dietitian.

Emotional and mental health support.

Patients with respiratory disorders may experience anxiety and depression. Getting needed professional mental health help can reduce the stress associated with managing a respiratory disorder. Additionally, support from family members and friends can help patients cope.

Family caregivers play an important role in managing a respiratory disorder.

  • Patients can feel isolated or anxious about their condition.
  • Adherence to therapy can be challenging without motivation.
  • Proper use of medical devices such as oxygen concentrators or nebulizers requires training.
  • Emotional encouragement and consistent routines improve mental health and recovery outcomes.

Family caregivers can offer support by:

  • Listening actively and without judgment.
  • Encouraging open conversations about fears.
  • Facilitating participation in a support group.
  • Helping the patient maintain hobbies and routines.

Emergency preparedness.

Every respiratory patient should have a clear action plan for flare-ups or emergencies. Your plan should include:

  • A list of emergency contacts.
  • Knowing when to use rescue inhalers or oxygen.
  • A ready-to-go hospital bag.
  • Up-to-date medical records and prescriptions.

Rehearsing the action plan regularly can help reduce panic during real emergencies.

Supporting pediatric vs. elder patients.

Pediatric care.

Children with asthma or congenital lung issues require:

  • Age-appropriate education.
  • Parental modeling of inhaler use.
  • Safe play environments free from triggers.

Elder care.

If you’re a family caregiver for a senior with respiratory disease, you may need to help your loved one with:

  • Mobility.
  • Changing positions to help improve breathing and prevent bedsores.
  • Proper use of medical devices.
  • Preventive care to reduce the risk of pneumonia and infections.

Community and social support resources.

Connecting with others facing similar challenges will make managing a respiratory disorder easier. By taking advantage of resources, you can receive reassurance, education, and maybe even financial aid.

You may find support through the following:

  • Virtual and/or in-person support groups for patients and family caregivers. Options include programs at  your hospital, American Lung Association’s Better Breathers Club, American Lung Associations’ Patient & Caregiver Network , COPD Foundation, and disease specific groups on Facebook.
  • Home health agencies for part-time assistance.
  • Subsidized oxygen therapy equipment may be available through nonprofit organizations.

Preventing caregiver burnout.

  • Set healthy boundaries.
  • Take breaks and rest regularly.
  • Seek respite care or support services.
